Participant Waiver

By completing this form, you acknowledge and agree to the following:

I acknowledge that I am voluntarily participating in a yoga, movement, breathwork, and/or sound-based session offered by Rolling Om Yoga Studio.

I understand that these practices may involve physical movement, stillness, and exposure to sound vibrations, and may carry certain inherent risks including, but not limited to:

  • muscle strain

  • dizziness

  • fatigue

  • emotional or physical discomfort

  • sensitivity to sound or vibration

I understand that I am responsible for listening to my body, modifying or resting as needed, and choosing what feels safe and appropriate for me.

I acknowledge that Rolling Om Yoga Studio and its instructor are not medical professionals and do not provide medical advice, diagnosis, or treatment. I understand that it is my responsibility to consult a physician or qualified healthcare provider regarding any medical conditions, injuries, or concerns before participating.

I affirm that I am participating voluntarily and at my own risk, and I accept full responsibility for my health and well-being during and after the session. I hereby release and hold harmless Rolling Om Yoga Studio and its instructor from any and all liability, claims, or demands arising from my participation, except in cases of gross negligence or willful misconduct.

I understand that I must complete this waiver before participating and that participation may be declined if this waiver is not completed in full before time of event.

Photography & Media Acknowledgment

I understand that photos or short video clips may occasionally be taken during sessions for promotional purposes by Rolling Om Yoga Studio. I understand that participant privacy is always respected. Identifying features will be avoided or blurred when possible. Full images or names will never be used without consent. If I prefer not to be photographed or recorded, I will notify the instructor prior to the session, and my preference will be respected.

Age and Guardian Consent

I affirm that I am at least 18 years of age. If I am under the age of 18, I understand that this waiver must be completed and signed by a parent or legal guardian on my behalf.

If signing as a parent or legal guardian, I acknowledge that I have read and agree to the terms of this waiver for the minor participant, and I consent to their participation in the session.
